Eligibility under the scheme is primarily based on residency status in Punjab. Applicants must be able to establish identity and residence through valid documents.
There is no income ceiling or occupation‑based classification for coverage. All eligible families can avail benefits under a single allotted health card.
To apply for the scheme and receive the associated health card, applicants are required to submit basic identification documents. Aadhaar Card is commonly used as proof of identity and residence.
A voter ID Card is also required during registration. A passport‑size photograph needs to be attached at physical registration centres.
Enrollment under the Mukh Mantri Sehat Yojna is supported through multiple registration modes. During the initial rollout, registration camps were organised in districts such as Tarn Taran and Barnala.
Residents could visit these camps and complete the application process onsite with assistance from officials. Similar camp‑based drives may be conducted in future phases.
Once the application process is completed, eligible families are issued a health card, either in physical or digital form. This card is commonly referred to as the Sehat Card.
The card is linked to the beneficiary’s Aadhaar details. It enables access to cashless medical treatment up to ₹10 lakh.
