India’s expansion of Ayushman Bharat to cover all citizens aged 70 and above is already reshaping access to healthcare, with millions of seniors registering and more than seven lakh treatments authorised.

Rising Enrolments and Impact of the Expanded Cover

Since the expansion in October 2024, Ayushman Bharat Pradhan Mantri Jan Arogya Yojana has enabled over 700,000 treatments worth ₹1,471 crore for elderly beneficiaries by October 2025.

The National Health Authority reported 9.26 million+ registrations in the 70+ category, of which 5.7 million were new entrants, and 3.4 million were seniors from families already covered under PMJAY.

The programme aims to eventually reach 60 million elderly citizens, offering each family ₹5 lakh cover, with an additional ₹5 lakh top-up for qualifying households.

Statewise Beneficiaries

State-level uptake varies widely. Uttar Pradesh leads with 1.85 million registrations, followed by Madhya Pradesh at 1.48 million, which has achieved 43% of its target.

Kerala recorded 708,041 new registrations, amounting to 41% of its estimated 70+ population. In contrast, enrolment has been slower in states like Tamil Nadu (206,640), Karnataka (202,348) and Telangana (26,439).

Ongoing Challenges and Support Measures

Despite strong momentum, overall coverage stands at only around 15% of the target elderly population. To accelerate enrolment, the NHA is strengthening hospital-level support by deploying Pradhan Mantri Arogya Mitras to assist with verification, card generation and claims.

The scheme also ensures that seniors with private insurance or ESIC coverage remain eligible, while those under CGHS or ECHS must choose between schemes. The separate Ayushman Vaya Vandana Card distinguishes senior beneficiaries under the expanded programme.

Conclusion

With millions already enrolled and treatment authorisations rising, the expanded Ayushman Bharat scheme is playing a pivotal role in easing financial stress for India’s elderly. Continued operational improvements and stronger outreach will be key to achieving the nationwide goal of covering 60 million senior citizens.
