
The Ministry of Health and Family Welfare (MoHFW) has issued a major clarification regarding dependency rules under the Central Government Health Scheme (CGHS). According to an office memorandum dated May 13, male central government employees can now exercise only a one-time choice between including either their parents or parents-in-law as dependents for CGHS medical benefits.
The ministry clarified that once the selection is made, it will be treated as final and irreversible. The employee will not be allowed to modify or switch the dependency option later, even if the selected dependent family members pass away in the future.
The move is aimed at reducing confusion surrounding dependency claims while ensuring smoother administration of CGHS benefits across government departments.
Under the revised clarification, male employees covered under CGHS or Central Services (Medical Attendance) Rules, 1944, must carefully choose which set of parents they want to include for healthcare coverage.
The government noted that the dependency option cannot be altered retrospectively or amended later. This makes the decision a significant long-term healthcare and financial consideration for employees and pensioners.
The rule follows earlier reforms promoting gender parity in government healthcare benefits. Female employees were first granted this option in 1987, while the same provision was extended to male employees in 2023. The policy was later incorporated into the Central Services (Medical Attendance) Rules in 2024.
As per existing CGHS norms, dependents can include spouses, children, parents, parents-in-law, widowed daughters, dependent siblings, and certain other relatives.
However, dependency eligibility is linked to income criteria. Family members are considered dependents only if their combined monthly income from all sources remains below ₹9,000, including pension and dearness allowance.
The only exception is the spouse of the CGHS beneficiary, who remains eligible even if receiving a family pension above the prescribed limit.
CGHS remains one of the most important healthcare schemes for central government employees and pensioners, offering access to medicines, diagnostic tests, OPD consultations, hospitalisation, and cashless treatment through empanelled hospitals and wellness centres.
The latest clarification is expected to improve transparency and reduce administrative complications related to dependency claims. It also encourages employees to make thoughtful long-term decisions while selecting beneficiaries under the scheme.
The government’s latest clarification on CGHS dependency rules marks an important step towards streamlining healthcare benefit administration for central government employees. While the one-time dependency choice offers greater policy clarity and consistency, it also places greater responsibility on employees to carefully assess their family’s long-term medical and financial needs.
